1. Log-in to the electronic declaration system of the State Tax Inspectorate (hereinafter – EDS) HERE (Lithuanian version)
2. Select ‘Deklaravimas’ in the top horizontal menu of EDS, then ‘Pildyti form’ in the left vertical menu.
3. Then, select ‘Prašymas skirti paramą’
4. Next, fill in the form FR0512 v.4 directly in the EDS portal.
5. On the form, please fill in your personal details and the following fields, (!) ticked fields must be filled in:
   (!) 5 Mokestinis laikotarpis – 2023
   (!) 6S Mokesčio dalį skiriu paramos gavėjams – ✔
   (!) E1 Gavėjo tipas – 2 [paramos gavėjas]
   (!) E2 Gavėjo identifikacinis numeris (kodas) – 300514378
   (!) E3 Mokesčio dalies paskirtis – VšĮ „Kino pavasaris“
   (!) E4 Mokesčio dalies dydis (procentais) – iki 1,2
   (!) E5 Mokesčio dalį skiriu iki mokestinio laikotarpio – vėliausiai iki 2027 m. [pildoma tik norint paramą skirti ilgiau kaip 1 metus]

1. Download the VMI mobile app from either the App Store or Google Play
2. Once downloaded, select EDS Declarations (Lithuanian) and log-in
3. Select both ‘Paramos skyrimas’ and ‘Pildyti naują’
4. Fill in the necessary fields for the grant according to the data above.
